Below are scheduled events where you can meet with an enrollment specialist. You can also make an individual appointment through Healthy Kids at 303-441-1589

What should I bring for my appointment?

• Picture ID and/or school ID if age 16 or older
• Passport or birth certificate
• Proof of income - pay stub or letter from employer
• Proof of legal residency if non-citizen
• Social security number
