What services are covered?

CHP+ and Medicaid are comprehensive health insurance programs and provide coverage for a wide range of services including:

Doctor visits
Hospital care
Vision services
Immunizations

Dental care
Mental health care
Prescriptions
Other health services

What is the cost?

Medicaid is free to eligible enrollees. CHP+ may have a premium of $25 a year for 1 child and $35 a year for 2 or more children. If you are assessed a family premium for CHP+ and need help paying please call Healthy Kids at 303.441.1589.

What should I bring for my appointment?

· Picture ID and/or school ID if age 16 or older

· Passport or birth certificate

· Proof of income - pay stub or letter from employer

· Proof of legal residency if non-citizen

· Social security number
